O'Connor has been signed on a three-year deal, working under reappointed head coach Richard Graham, who was re-signed for just one season.
Reportedly one of those in the shortlist for the head coaching position, O'Connor applied specifically for the attacking job once Graham's appointment was announced.
The former Brumbies assistant comes to the Reds from a stint with Irish club Leinster.
"Ballymore is one of the true rugby centres in Australia that still exists. It's exciting to join an organisation with such great rugby pedigree, history and tradition," O'Connor said in a statement.
"My family has been overseas for 13 of the last 17 years, so it's huge to be able to return to Australia and we're looking forward to making the move to Queensland to join the Reds."
O'Connor is the first appointment made by the newly installed Reds general manager, former Wallaby Dan Herbert.
Herbert came on board as part of the Queensland Reds' "wide-ranging" review of its entire rugby operations, with the announcement of Graham as head and defence coach.
O'Connor's name was frequently linked to the Reds head coaching position, among a handful of overseas-based coaches.
The Reds attack was poor in a four-win 2015 and Herbert said he was confident that O'Connor could turn it around.
"The Reds rugby panel considered a number of local and international candidates to find someone who could help us deliver the best rugby program in Australia," Herbert said.
"Matt has a proven track record. He has a history of improving rugby programs with a strong attack-based approach to his coaching.
"We're looking forward to him making a significant contribution to Queensland rugby."
O'Connnor joins Graham and Nick Stiles (senior assistant) as the key Reds coaches for 2016.
