Late tries to Riley Manwaring and Will Manwaring and a match-winning sideline conversion from captain Nick Bush lifted Young to victory at Alfred Oval.
It's Young's first reserve grade minor premiership since 2004 - the same year they went on to win the premiership.
The result means Young is one win away from a grand final berth thanks to automatic qualification into the major semi-final in a fortnight.
Young trailed 10-4 at halftime and faced a 14-4 deficit for most of the second half before storming home late to overcome a strong Bulldogs outfit.
Nick Bush was hugely relieved to cap the regular season by winning the minor title.
"It's so good. It's been a long time coming for this group. There's been a lot of hard work, we deserve it. It's phenomenal I can't describe it," Bush said.
"That win is huge especailly against a Cootamundra side that had a couple of first graders."
Sam Norton scored the opening try to give Young an early lead, however it was short lived as the Bulldogs crossed twice through Luke Levett and Brad Walsh to hold a 10-4 advantage at the break.
Patrick Cameron extended Cootamundra's lead to 14-4 with a long-range effort shortly after the restart.
The score didn't change until about eight minutes until fulltime when Riley Manwaring picked up a loose ball and ran 80 metres.
Young had the momentum and crossed again through Will Manwaring who barged across on the right edge on the western sideline.
With his bench, coaches and team officials behind him, Bush successfully added the extras to give Young a two-point win.
"Without those two individual efforts I wouldn't have had the chance," Bush said referring to the tries scored by Riley and Will Manwaring.
"You put in all the effort in since under 12s and practice for a chance like that. Especially at home to kick a goal to make sure we win the minor premiership. It is something you practice for. You come up here on a Tuesday afternoon and kick for two hours hoping you get to something half as good as that."
Bush said his side needs to now focus on winning the premiership.
"We'll go watch the girls or go watch the juniors as a team [next weekend]. And we'll get back to training, no weeks off, we have to earn it," he said.
