Emergency services responded to calls after a Holden Statesmen hit a tree on the Henry Lawson Way about 40 kilometres south of Forbes, between Glenelg and Ooma, around 3.20pm on Saturday, September 22, 2018.
The passenger of the vehicle, a girl aged seven at the time, was airlifted to The Children's Hospital at Westmead in a critical condition while the driver, a then 43-year-old woman, had minor injuries treated by paramedics at the scene.
Officers attached to Central West Police District attended and commenced an investigation into the circumstances surrounding the crash, with assistant from the Crash Investigation Unit.

In March of 2019 the 43-year-old woman was charged with dangerous drive occasioning grievous bodily harm - drive manner dangerous and negligent driving (occasions grievous bodily harm). She remains before the courts.
In October of 2019, the girl died in hospital.
Following further inquiries officers from Central West Crash Investigation Unit laid an additional three charges on Tuesday, February 8; dangerous driving occasioning death - drive under influence, dangerous driving occasioning death - drive manner dangerous and negligent driving (occasioning death).
The woman is due to appear at Young Local Court on Tuesday.
